1 Phase 1: Define & Measure
Identify the process to improve with specific scope (e.g., "ED patient discharge" not "improve patient flow")
Document the current-state process on a canvas — every step, handoff, and wait time
Collect baseline metrics: cycle time, error rate, patient satisfaction score, staff time per case
Identify the specific metric you want to improve and set a target (e.g., "reduce discharge time from 4.2 hours to 2.5 hours")
Map stakeholders: clinical staff, nursing, admin, compliance, IT — create a card for each
Check regulatory constraints: Joint Commission, CMS, state health department requirements that apply to this process
2 Phase 2: Analyze & Design
Run a root cause analysis — use the canvas to map contributing factors to outcome cards
Identify the 3 largest time or error contributors using data from baseline measurement
Design the future-state process on a separate canvas zone — add/remove/modify steps
Draw connectors from the changed steps to the downstream impacts
Review the future-state with frontline staff (nurses, techs, clerks) — incorporate their feedback
Validate compliance: confirm the new process meets HIPAA, Joint Commission, and CMS requirements
Use AI Dependency Detection to identify steps where the change creates new dependencies
3 Phase 3: Pilot
Select a pilot unit or shift (e.g., "3rd floor medical-surgical, day shift, for 2 weeks")
Train pilot staff on the new process — create training cards with the specific changes
Collect the same baseline metrics during the pilot for comparison
Track deviations: when staff revert to the old process, document why in card notes
Hold daily 10-minute check-ins during the first week of the pilot
Run AI Health Check at pilot midpoint to assess whether the checklist cards are being completed on schedule
4 Phase 4: Evaluate & Scale
Compare pilot metrics to baseline — calculate actual improvement percentage
If the target was met, plan rollout to remaining units with a phased timeline
If the target was missed, identify which process steps underperformed and revise
Update standard operating procedures (SOPs) and training materials
Create a sustainability plan: who monitors the metrics monthly, what triggers re-intervention
5 Phase 5: Sustain
Set a 30-day, 60-day, and 90-day measurement milestone on the canvas
Assign a process owner who reviews metrics at each milestone
Run a canvas snapshot at each milestone to track whether gains are holding
If metrics regress more than 10%, trigger a mini-PDCA cycle using the canvas
Archive the completed improvement project canvas as a template for future initiatives
